Dementia Care Family Support Pilot Program Available
Resolve three care challenges during five sessions with a Certified Caregiving Consultant.
When you research a disease process like dementia, you learn general information.
What if you could receive personalized insights based on your caree’s everyday function level?
With that personalized information, imagine having care plans tailored just for your situation. Because of those tailored care plans, you have strategies that create an easier day for you.
That's just the solution we're launching. Our Certified Caregiving Consultants will support dementia family caregivers by complementing their insights and tools with evidence-based strategies to address your care challenges.
We are partnering with Plans4Care Inc. to provide you with the best evidence-based strategies that are easy to use to help you. Through our partnership, we are launching Dementia Care Family Support Pilot Program.
Through the pilot program, you will meet with the CCC of your choice for five free sessions to:
complete an Everyday Function Scale to better understand your caree’s abilities
complete an assessment to better understand your own needs
share three care challenges such as behavioral symptoms or finding time for your own self-care
create care plans to manage these three daily challenges
connect to determine if any tweaks to the care plan are needed.
